Allopurinol【C】

Relief of Pain and Inflammation : Gout

適應症:痛風症、痛風性關節炎、尿酸結石、癌症或經化學治療產生之高尿酸血症。

Usual dose:

Calcium renal calculus:

Adult: 200-300 mg qd or divided dose bid-tid, max. 300 mg/dose, 800 mg/day.

Cancer – Hyperuricemia:

Adult: 600-800 mg/day, 12 h-3 days prior to initiation of chemotherapy.

Pediatric (< 6 yrs): 50 mg tid.

Pediatric (6-10 yrs): 100 mg tid or 300 mg qd.

Gout (mild):

Adult: 100-300 mg/day qd or divided dose bid-tid.

Gout (moderate to severe):

Adult: 400-600 mg/day qd or divided dose bid-tid; max. 800 mg/day.

Dose adjustment:

Maintenance dose should be based on serum uric acid determinations performed 48 h after initial dose.

Renal impairment: CrCl 10-20 ml/min, 200 mg daily; CrCl 3-10 ml/min, 100 mg daily; CrCl < 3 ml/min, 100 mg at extended intervals > q24h.

Precaution:

Discontinue at first sign of allergic reaction.

During concommitant therapy, reduce doses of azathioprine and mercaptopurine to 1/3 to 1/4 of the usual dose.

Maintain fluid intake necessary to yield a daily urine output of at least 2 liters in adults.

Adverse effect:

Common: pruritus, rash, nausea, vomiting, renal failure

Serious: Stevens-Johnson syndrome, agranulocytosis, anemia, myelosuppression, hepatotoxicity.

1.限使用於痛風症、痛風性關節炎、尿酸結石、癌症或經化學治療產生之高尿酸血症之治療。

2.本藥品可能發生嚴重藥物不良反應,故不得使用於非核准之適應症。且須充分提醒病人相關不良反應症狀及後續作為。
