Initial Therapy
The initial recommended dose is amlodipine 5 mg/valsartan 160 mg qd and titrate every 1 to 2 weeks, if necessary, to a max. dose of amlodipine 10 mg/valsartan 320 mg daily. The optimum effect is attained within 2 weeks of starting therapy or changing the dose.
Add-On-Therapy
The recommended dose is amlodipine 5 mg/valsartan 160 mg to amlodipine 10 mg/valsartan 320 mg orally once daily. Titrate every 1 to 2 weeks, if necessary, to a max. dose of amlodipine 10 mg/valsartan 320 mg daily.
For patients who experienced dose-limiting adverse effects on either agent alone, switch to a lower dose of that agent in combination with the other to attain similar blood pressure reductions and make subsequent dose adjustments after 3 to 4 weeks to a maximum of amlodipine 10 mg/valsartan 320 mg. The optimum effect is attained within 2 weeks of starting therapy or changing the dose.
Dose adjustment:
Geriatric: amlodipine 2.5 mg should be the initial dose.
Hepatic failure: starting dose amlodipine 2.5 mg qd.
Precaution:
Increased risk for fetal and neonatal toxicity and death when used during pregnancy, especially during the second and third trimester; discontinue therapy as soon as pregnancy is confirmed.
Adverse effect:
Common: Peripheral edema, dizziness, nasopharyngitis, upper respiratory infection.
Serious: Hypotension.
