Anti-infective Agents : Penicillins

OAMOX5 “Amoxicillin capsule” 500 mg/cap
OAMOX6 “Amoxicillin capsule” 250 mg/cap
適應症：葡萄球菌、鏈球菌、肺炎雙球菌、腦膜炎球菌，及其它具有感受性細菌引起之感染。
Usual dose:
Susceptible infections: mild-to-moderate
Adult: 500 mg q12h or 500 mg q8h.
Pediatric
(≦3 mon): 30 mg/kg/day orally divided q12h.
(> 3 mon): 25-50 mg/kg/day orally divided q8h.
Susceptible infections: severe
Adult: 875 mg q12h or 500 mg q8h; max. 1.5gm/day.
Pediatric
(≦3 mon):, 30 mg/kg/day orally divided q12h.
(> 3 mon): 25-50 mg/kg/day orally divided q8h.
Endocarditis prophylaxis:
Adult: 2 gm orally 1 h prior to procedure.
Pediatric: 50 mg/kg 1 h prior to procedure; max. 2 gm/dose.
Gonorrhea: 3 gm as a single dose.
Helicobacter pylori: 1 gm bid in combination with clarithromycin 500 mg bid; or 1 g tid.
Dose adjustment:
Renal failure: CrCl 10-30 ml/min, usual dose q12h; CrCl < 10 ml/min, usual dose q24h.
Hemodialysis: usual dose q24h (give additional dose both during and at the end of dialysis).
Adverse effect:
Common: rash, diarrhea, nausea, vomiting.
Serious: hypersensitivity reaction.
