適應症:嚴重度高血壓、腎血管性高血壓、以及對傳統治療之高血壓無理想效果或發生不良副作用之病人
Usual dose:
Congestive heart failure, usually in combination with diuretics and digitalis:
Adult: initial, (p’ts with normal or low blood pressure, vigorous diuretic therapy, volume depletion) 6.25-12.5 mg tid; initial, 25 mg tid, maintenance, 50-100 mg tid, max. 450 mg daily.
Hypertension:
Adult: 25 mg bid-tid; may be increased after 1-2 wk to 50 mg bid-tid, then to 100-150 mg bid-tid (in combination with a thiazide diuretic) if needed, max. 450 mg daily.
Left ventricular dysfunction, following myocardial dysfunction:
Adult: initial dose, 6.25 mg for one dose, first titration, 12.5 mg tid for several days, second titration, 25 mg tid for several days; titrate up to 50 mg tid over next several wks as tolerated, target dose, 50 mg tid.
Dose adjustment:
Renal impairment: initial dose should be reduced; titration should be in smaller increments
Contraindication: history of angioedema.
Adverse effect:
Common: edema, hypotension, rash, gynecomastia, hyperkalemia, taste sense altered, proteinuria, cough.
Serious: angioedema in face, lips, throat (1/1000 p’ts; more frequent in black p’ts); intestinal angioedema, agranulocytosis, neutropenia.
