適應症：
ODILA2：鬱血性心臟衰竭。
OLONG1, OSYNT5：高血壓、鬱血性心臟衰竭。
Usual dose:
Angina: 25-50 mg bid.
Congestive heart failure: initial, 3.125 mg bid for 2 wks, dose titration may be doubled q2w to highest dose tolerated; < 85 kg: max. 25 mg bid, > 85 kg: max. 50 mg bid.
Hypertension: initial, 6.25 mg bid, dose titration may be doubled q1-2w, max. 25 mg bid.
Impaired left ventricular function- Myocardial infarction: Initial 6.25 mg bid, titration, dose may be increase after 3-10 days to 12.5 mg bid, then again to target/max 25 mg bid.
Dose adjustment:
Hepatic failure: initial, 20% of the normal dose.
Contraindication:
Bronchial asthma or chronic obstructive pulmonary disease, cardiogenic shock, overt cardiac failure, second and third degree AV block, severe liver failure, severe sinus bradycardia, sick sinus syndrome.
Adverse effect:
Common: angina, atrioventricular block, bradycardia, edema, hypotension, palpitations, syncope, fatigue, dizziness, headache, insomnia, somnolence, hyperglycemia, abdominal pain, diarrhea, nausea, vomiting, erectile dysfunction, reduced libido, visual abnormalities, bronchospasm, dyspnea, pharyngitis, rhinitis, skin rash, pruritus, myalgia, back pain.
Serious: hepatotoxicity, status asthmaticus, thrombocytopenia.
