適應症：葡萄球菌、鏈球菌、肺炎雙球菌、腦膜炎球菌及其他具有感受性細菌所引起之感染症。
Usual dose:
Mild infections: IV/IM, 250-500 mg q8h.
Moderate to severe infections: IV/IM, 0.5-1 gm q6-8h.
Severe, life-threatening infections: IV/IM, 1-1.5 gm q6h.
Pneumococcal pneumonia: IV/IM, 500 mg q12h.
Pre-operative use: IV/IM, 1 gm 0.5-1 h prior to surgery followed by 0.5-1 gm q6-8h for 24 h.
UTI, uncomplicated: IV/IM, 1 gm q12h.
Perinatal GBS disease, intrapartum prophylaxis: IV, 2 gm initially (started at time of labor or rupture of membranes) then 1 gm q8h until delivery.
Susceptible infections: IV/IM
Pediatric(≧1 mon): 50-100 mg/kg/day divided q8h, max. 6 gm/day.
Dose adjustment:
Renal failure: CrCl 35-54 ml/min, usual dose q8h; CrCl 10-34 ml/min 50% usual dose q12h; CrCl < 10 ml/min 50% usual dose q18-24h.
Adverse effect:
Common: diarrhea, nausea, vomiting.
Serious: thrombocytopenia, leukopenia, pseudomembranous colitis, hepatotoxicit, Stevens-Johnson syndrome.
