Anti-infective Agents : Macrolides

適應症：
OKLAR4：治療下列有感受性的微生物所引起之感染症：1.上呼吸道感染（如:鏈球菌咽炎 Streptococcal pharyngitis）；2.下呼吸道感染（如:細枝氣管炎、肺）；3.急性中耳炎；4.皮膚及皮膚結構感染（如:膿庖病、毛囊炎、蜂窩組織炎、膿瘍）。
OKLAR1：上、下呼吸道有感受性細菌所引起之感染症。可與omeprazole併用治療與幽門螺症桿菌(H.Pyori)有關之十二指腸潰瘍。
Usual dose:
Adult: 250-500 mg q12h.
Helicobacter pylori infection:
Adult: 500 mg q8-12h.
Pediatric: 7.5 mg/kg bid; max. 500 mg bid.

Contraindication: concomitant cisapride, pimozide, astemizole, terfenadine.
Dose adjustment:
Renal failure: CrCl < 30 ml/min, dose should be halved or the dosing interval doubled.
Adverse effect:
Common: abdominal discomfort, abdominal pain, diarrhea, disorder of taste, dyspepsia, nausea, headache.
Serious: liver failure, allergic reaction, anaphylaxis, Stevens-Johnson syndrome, toxic epidermal necrolysis.
健保使用規範：(90/11/1、93/9/1、97/12/1、98/10/1、100/5/1)
- 限用於經臨床診斷或實驗室診斷為黴漿菌 (mycoplasma) 或披衣菌 (chlamydia) 或退伍軍人桿菌 (legionella) 引起之感染，或經培養證實為macrolides有效之致病菌感染（需於病歷記載診斷依據，俾利審查）。
2.Clarithromycin (如Klaricid Tab、Klaricid Paediatric Suspension)使用期間不得超過10日，每日最大劑量500mg。對於「非結核分枝(耐酸)桿菌 (NTM) 」感染患者，每日得使用1000mg，且得持續使用6個月以上。(93/9/1、100/5/1)
3.Clarithromycin (如Klaricid Tab) 用於消化性潰瘍之胃幽門桿菌消除治療，使用總量以28顆（每顆250mg）為限；依比例換算使用clarithromycin 500mg者，則使用總量以14顆為限。(98/10/1、100/5/1)
- 本類製劑應儘量避免合併使用其他抗生素，用於治療「非結核分枝(耐酸)桿菌 (NTM)」者不在此限。(100/5/1)
