適應症：
ICLIN1：葡萄球菌、鏈球菌、肺炎雙球菌、厭氣菌等具有感受性菌株所引起之感染症。
OLIND1：鏈球菌、葡萄球菌、肺炎球菌等感染治療。
Usual dose:
Adult: IV/IM 0.6-1.2 g/day divided q6-12h; severe, 1.2-2.7 g/day divided q6-12h; max. 4.8 g/day. PO 150-300 mg q6h; severe, 300-450 mg q6h.
Pediatric: IV/IM 15-20 mg/kg/day divided q6-8h; severe, 20-40 mg/kg/day divided q6-8h; max. 4.8 g/day.
Pediatric
(≧ 10 kg): 8-25 mg/kg/day depending on severity of infection given in 3-4 divided doses.
(< 10 kg): 37.5 mg q8h.
Precaution:
IV should dilute to concentration≦18 mg/ml and give no faster than 30 mg/min; single IM doses≦600 mg.
Dose adjustment:
Hepatic failure: severe liver disease, give q8h.
Adverse effect:
Common: dry skin, peeling of skin, pruritus, diarrhea, nausea.
Serious:pseudomembranous enterocolitis, jaundice, liver function tests abnormal.
