適應症:
OLANO1:心臟衰弱症。
ILANO1, OCARD3:心臟衰竭、心房撲動、心房纖維顫動、陣發性上室性心搏過速。
Usual dose:
Gradual digitalization: PO
Adult: 0.125-0.5 mg qd for 7 days.
Gradual digitalization: IV
Adult: initial, 0.4-0.6 mg, followed by 0.1-0.3 mg q4-6h as needed; maintenance, 0.125-0.5 mg qd or divided.
Rapid digitalization: PO
Adult: 0.75-1.25 mg in 2 or more divided doses; maintenance: 0.125-0.5 mg qd or divided.
Pediatric: IV/PO, give in divided doses.
(premature): IV 15-25mcg/kg.
(full-term): IV 20-30 mcg/kg.
(1-24 mon): IV 30-50 mcg/kg.
(2-5 yrs): IV 25-35 mcg/kg; maintenance, PO 10-15 mcg/kg.
(5-10 yrs): IV, 15-30 mcg/kg; maintenance, PO 7-10 mcg/kg.
(>10 yrs): IV, 8-12 mcg/kg; maintenance, PO 3-5 mcg/kg.
Supraventricular arrhythmia:
Adult: 8-12 mcg/kg.
Dose adjustment:
Renal failure: initial 0.125 mg daily; severe impairment, initial 0.0625 mg daily; titrate dose q2w.
Geriatric: (> 70 yrs & good renal function): initial, 0.125 mg qd.
Contraindications: ventricular fibrillation.
Adverse effect:
Common: diarrhea, loss of appetite, nausea and vomiting, headache, visual disturbance.
Serious: cardiac dysrhythmia.
