Cardiovascular – Renal Drugs : Agents used to Treat Circulatory Failure

(1) 不可與NaHCO3混合
(2) 通常為最後一線使用,但若合併bradycardia才可考慮第一線使用
適應症:休克症候群及心臟衰竭。
Usual dose:
Shock, heart failure: IV
Adult: initial, 2-5 mcg/kg/min IV; increase in 5-10 mcg/kg/min increments; max. 50 mcg/kg/min IV.
Hypotension, acute:
Pediatric: initial, 2-5 mcg/kg/min IV; increase in 5-10 mcg/kg/min increments; max. 30 mcg/kg/min IV.
Dose adjustment:
P’ts who have been receiving MAOIs prior to the use of dopamine will require a dosage reduction.
Contraindication: pheochromocytoma.
Precaution:
Do not add dopamine to sodium bicarbonate or any other strongly alkaline solution
Tachyarrhythmias, ventricular fibrillation.
Adverse effect:
Common: anxiety, headache chest pain, hypertension, palpitations, tachycardia dyspnea injection site reactions mydriasis nausea, vomiting oliguria piloerection.
Serious: ectopic heartbeats, gangrene of extremities widened QRS complex, ventricular arrhythmias.
