IEASY1 “Easydopa injection ”
Each ml contains dopamine HCl 1.6 mg, dextrose hydrous 50 mg; 500 ml/bag
適應症:休克症候群及心臟衰竭。
Usual dose:
Bradyarrhythmia, acute symptomatic: 2 to 10 mcg/kg/min IV and titrate to response
Cardiac arrest – Hypotension, acute:
Adult: 5 to 10 mcg/kg/min IV, titrate to response.
Pediatric: 2 to 20 mcg/kg/min IV/INTRAOSSEOUS, titrate to response
Cardiogenic shock: initial, 2 to 5 mcg/kg/min IV; increase in 5 to 10 mcg/kg/min increments; max 50 mcg/kg/min IV.
Congestive heart failure: initial, 2 to 5 mcg/kg/min IV; increase in 5 to 10 mcg/kg/min increments; max 50 mcg/kg/min IV.
Decreased cardiac output: initial, 2 to 5 mcg/kg/min IV; increase in 5 to 10 mcg/kg/min increments; max. 50 mcg/kg/min IV.
Renal failure: initial, 2 to 5 mcg/kg/min IV; increase in 5 to 10 mcg/kg/min increments; max 50 mcg/kg/min IV.
Septic shock: initial, 2 to 5 mcg/kg/min IV; increase in 5 to 10 mcg/kg/min increments; max 50 mcg/kg/min IV.
Shock, due to MI, trauma, or open heart surgery: initial, 2 to 5 mcg/kg/min IV; increase in 5 to 10 mcg/kg/min increments; max. 50 mcg/kg/min IV.
Hypotension, acute:
Pediatric: initial, 2 to 5 mcg/kg/min IV; increase in 5 to 10 mcg/kg/min increments; MAX 30 mcg/kg/min IV
Dose adjustment:
MAOI use: patients who have been receiving MAOIs prior to the use of dopamine will require a dosage reduction (specific guidelines unavailable).
Contraindication: hypersensitivity to dopamine, pheochromocytoma, tachyarrhythmias/ventricular fibrillation
Precaution:
Do not add any other strongly alkaline solution to bag.
Adverse effect:
Common: chest pain, hypertension, palpitations, tachyarrhythmia, injection site reaction, piloerection, nausea, vomiting, headache, mydriasis, anxiety, oliguria, dyspnea.
Serious: ectopic beats, gangrenous disorder, ventricular arrhythmia, wide QRS complex.
