適應症:高血壓、充血性心臟衰竭

Usual dose:

Congestive heart failure:

Adult: initial, 2.5 mg qd or bid; maintenance, 2.5-20 mg bid; max. 20 mg daily in divided doses.

Pediatric (≧4 yrs): 0.1-0.5 mg/kg qd or in 2 divided doses, max. 0.94 mg/kg/day.

Hypertension:

Adult: initial, 5 mg qd; maintenance, 10-40 mg qd or in 2 divided doses.

Pediatric (1 mon-16 yrs): initial, 0.08 mg/kg qd (up to 5 mg), max. 0.58 mg/kg or 40 mg/day.

Kidney disease, Nondiabetic:

Adult: initial, 5 mg qd; maintenance, 5-20 mg qd, max. 20 mg/day.

Dose adjustment:

Renal impairment: hypertension with renal impairment: CrCl≦30 ml/min (serum creatinine≧3 mg/dL), starting dose 2.5 mg qd; titrate to effect, max. 40 mg/day.

CHF with renal impairment or hyponatremia: starting dose 2.5 mg qd; titrate at 4-day intervals to 2.5 mg bid, then 5 mg bid, max. 40 mg/day.

Contraindication:

Angioedema, hereditary or idiopathic, angioedema related to prior therapy with an ACE inhibitor.

Adverse effect:

Common: hypotension, rash, hyperkalemia, nausea and vomiting, asthenia, dizziness, cough.

Serious: syncope, intestinal angioedema, liver failure, angioedema in face, lips, throat.
