Anti-infective Agents : Aminoglycosides
IVGEN1 “V-Genta injection”40 mg/ml; 2 ml/vial
適應症：革蘭氏陽性及陰性菌、立克次氏體，及巨型濾過性病毒感染症。
Usual dose: IV/IM
Adult: traditional dosing, 3-6 mg/kg/day divided q8h, once-daily dosing, 4-7 mg/kg/day IV q24h.
Pediatric
(neonates, < 28 wks gestational age): 2.5 mg/kg q24-36h.
(neonates, 28-32 wks gestational age): 2.5 mg/kg q18h.
(neonates, 33-42 wks gestational age): 2.5 mg/kg q12h.
(term neonates >1 wk, infants and children < 5 yrs): 2.5 mg/kg q8h.
(≧5 yrs): 2-2.5 mg/kg q8h.

Adverse effect:
Neuromuscular blockade finding, ototoxicity, nephrotoxicity, respiratory tract paralysis (concomitant anesthesia, muscle relaxants).

(1) Select loading dose in mg/kg(IBW) to provide peak plasma levels in the range listed below for the desired aminoglycoside.

(2) Select maintenance dose (as percentage of loading dose) to continue peak plasma levels indicated above, according to desired dosing interval and the p’t’s corrected creatinine clearance.
