適應症:
IPEDE1:對體重介於500和1500克,不超過懷孕週期32週,臨床顯著動脈導管閉鎖不全(PDA)的早產兒,且傳統治療無效時使用。
OIDOF1, OPURF1:慢性風濕關節炎、關節痛及關節炎、神經痛及神經炎、背腰痛等疾患及症狀之消炎、鎮痛;手術及外傷後之消炎、鎮痛。
Usual dose:
Fever, Pain: PO
Adult: 200-400 mg q4-6h as needed; max. 1200 mg/day.
Pediatric
(6 mon-12 yrs.): 5-10 mg/kg q6-8h as needed, max. 4 doses/day.
(> 12 yrs.): 200-400 mg q4-6h as needed, max. 1200 mg/day.
Osteoarthritis: PO
Adult: 1200-3200 mg/day divided in 3-4 doses.
Rheumatoid arthritis: PO
Adult: 1200-3200 mg/day divided in 3-4 doses.
Pediatric: 30-50 mg/kg/day in 3-4 divided doses; max. 2.4 g/day.
Patent ductus arteriosus: IV
Usual dose: 10 mg/kg IV over 15 minutes on day 1, followed by 5 mg/kg IV over 15 minutes after 24 and 48 hours for a total of 3 doses; doses are based on birth weight.
Study dosing: 10 mg/kg continuous IV infusion (0.416 mg/kg/hr) on day 1, followed by 5 mg/kg continuous IV infusion (0.208 mg/kg/hr) on days 2 and 3.
If ductus arteriosus fails to close or reopens, a second course of ibuprofen lysine, alternative pharmacological therapy, or surgery may be necessary.
Dose adjustment:
Renal impairment: initiate with the lowest recommended dosage.
Contraindication: See Diclofenac Potassium.
Adverse effect:
Common: body fluid retention, rash, abdominal pain, constipation, diarrhea, dyspepsia, heartburn, nausea, stomatitis, vomiting, increased liver function test, dizziness, headache, somnolence, tinnitus.
Serious: congestive heart failure, hypertension, myocardial infarction, thrombotic tendency observations, erythema multiforme, scaling eczema, Stevens-Johnson syndrome, toxic epidermal necrolysis, gastrointestinal hemorrhage, gastrointestinal perforation, gastrointestinal ulcer, inflammatory disorder of digestive tract, melena, pancreatitis, agranulocytosis, anemia, hemolytic anemia, neutropenia, thrombocytopenic disorder, hepatitis, jaundice, anaphylactoid reaction, aseptic meningitis, cerebrovascular accident, confusion, amblyopia, hearing loss, depression, acute renal failure, hematuria, renal azotemia.
