Hematological Agents : Deficiency Anemia
ICOVO1 “Covorin injection” 10 mg/ml; 5 ml/vial
IFOLI1 “Folina injection” 10 mg/ml; 5 ml/vail
適應症:抗葉酸拮抗劑。
Usual dose:
Antimetabolite overdose, folic acid antagonist: IM/IV
15 mg (10 mg/m2) q6h until serum methotrexate (MTX) level < 0.01 (mcmol); if 24 h serum creatinine 50% above baseline, 24 h MTX level above 5 mcmol, or 48 h MTX level above 0.9 mcmol, increase dose to 100 mg/m2 IV q3h until serum MTX < 0.01 mcmol.
Colorectal cancer, advanced, in combination with 5-FU (palliative treatment): IV
leucovorin 200 mg/m2 IV over a minimum of 3 min, followed by 5-FU 370 mg/m2 IV daily for 5 days; may repeat in 4-wk intervals for 2 courses then 4- to 5-wk intervals; adjust based on p’t tolerance.
Megaloblastic anemia due to folate deficiency, when oral therapy is not feasible: up to 1 mg IM/IV daily.
Adverse effect:
Common: diarrhea, nausea, stomatitis, vomiting, fatigue
Serious: allergic sensitization.
