適應症:心室性不整律之急性治療。
Usual dose:
Ventricular arrhythmia: IV
Adult: loading dose: 50-100 mg over 2-3 min, may repeat in 5 min up to 300 mg in any 1-h period; maintenance: 0.02-0.05 mg/kg at a rate of 1-4 mg/min.
Pediatric: loading dose 1 mg/kg IV/intraosseous, max. 100 mg; infusion, 20-50 mcg/kg/min.
Ventricular fibrillation: IV
Adult: initial dose: 1-1.5 mg/kg IV/intraosseous; if ventricular fibrillation/pulseless ventricular tachycardia persists, additional doses of 0.5-0.75 mg/kg IV/intraosseous q5-10 min to max. 3 mg/kg can be given.
Pediatric: bolus, 1 mg/kg IV/intraosseous, max. 100 mg; infusion, 20-50mcg/kg/min.
Dose adjustment:
Hepatic failure: dosage adjustments may be required.
Geriatric (>65 yrs): reduce loading dose by approximately 50% of normal dose and rate of infusion by half; adjust dose slowly.
Adverse effect:
Common: hypotension, injection site pain, constipation, nausea, vomiting, confusion, dizziness, headache, paresthesia, somnolence, tremor, irritation symptom, erythema, edema.
Serious: cardiac arrest, cardiac dysrhythmia, heart block, methemoglobinemia, loss of consciousness, seizure, tremor.
