Relief of Pain and Inflammation : Non-Steroidal Anti-inflammatory Agents (NSAID)
ONATO1 “Naton SR tablet” 750 mg/tab
適應症：急慢性風濕關節炎、骨關節炎、粘連性脊椎炎、關節周圍炎、上脕肩甲骨炎及骨骼肌不適之粘液囊炎、腱鞘炎之消炎、鎮痛、解熱。
Usual dose:
Pain, Bursitis, Tendinitis:
Adult: initial, 500 mg; maintenance, 250 mg q6-8h as needed or 500 mg q12h as needed; max initial 1250 mg/day, then 1000 mg/day
Acute gout:
Adult: initial, 750 mg; maintenance, 250 mg q8h.
Ankylosing spondylitis, Osteoarthritis, Rheumatoid arthritis:
Adult: 250-500 mg bid; max. 1500 mg/day for up to 6 mon. delayed-release, 375-500 mg bid; may increase to 1500 mg qd for a limited time; use lowest effective dose
Rheumatoid arthritis:
Pediatric: 10 mg/kg/day given in 2 divided doses (5 mg/kg bid)
Dose adjustment:
Geriatric: initiate treatment at the lowest end of the dosing range and when higher doses are indicated, a dose adjustment may be necessary
Renal impairment, moderate to severe (CrCl less than 30 mL/min): use not recommended
Contraindication: See Diclofenac Potassium.
Adverse effect:
Common: edema, ecchymosis, pruritus, rash, abdominal pain, constipation, heartburn, nausea, dizziness, headache, somnolence, ototoxicity, tinnitus, dyspnea.
Serious: congestive heart failure, vasculitis, erythema multiforme, generalized exfoliative dermatitis, Stevens-Johnson syndrome, toxic epidermal necrolysis, gastrointestinal hemorrhage, gastrointestinal perforation, gastrointestinal ulcer, hematemesis, inflammatory bowel disease, pancreatitis, agranulocytosis, aplastic anemia, granulocytopenic disorder, hemolytic anemia, thrombocytopenia, thrombosis, hepatic necrosis, hepatitis, increased liver function test, jaundice, liver failure, anaphylactoid reaction, aseptic meningitis, seizure, acute renal failure, nephritis, nephrotic syndrome, acute renal failure, nephritis, nephrotic syndrome, papillary necrosis, renal failure, bronchospasm, pulmonary edema, angioedema.
健保使用規範：
1.本類製劑之使用需符合下列條件之一者(99/10/1)：(1)年齡大於等於六十歲之骨關節炎病患。(2)類風濕性關節炎、僵直性脊髓炎、乾癬性關節炎等慢性病發炎性關節病變，需長期使用非類固醇抗發炎劑者。(3)合併有急性嚴重創傷、急性中風及急性心血管事件者 (97/2/1)。(4)同時併有腎上腺類固醇之患者。(5)曾有消化性潰瘍、上消化道出血或胃穿孔病史者。(6)同時併有抗擬血劑者。(7)肝硬化患者。2.使用本類製劑之病患不得預防性併用乙型組織胺受體阻斷劑、氫離子幫浦阻斷劑及其他消化性潰瘍用藥，亦不得合併使用前列腺素劑（如misoprostol）3.Nimesulide限用於急性疼痛緩解，其連續處方不得超過15日(97/9/1)。4.含naproxen及esomeprazole複方製劑不得作為急性疼痛的初始治療。(101/10/1)
