Cardiovascular – Renal Drugs : Hyperlipidemia
OCRES1 “Crestor tablet” 10 mg/tab
適應症:高膽固醇血症、高三酸甘油酯血症。
Usual dose:
Dyslipidemia, mixed (Fredrickson type IIa and type IIb hyperlipoproteinemia), generalized atherosclerosis, hypertriglyceridemia, primary hypercholesterolemia:
Initial, 5-10 mg qd; consider 20 mg qd for p’ts with LDL > 190 mg/dL and when aggressive lipid targets are desired; maintenance, dose range is 5-40 mg qd; the 40 mg dose should be reserved for p’ts who failed therapy with the 20 mg dose, max 40 mg/day.
Familial hypercholesterolemia – homozygous:
Initial, 20 mg qd; maintenance, dose range is 20-40 mg qd, max 40 mg/day.
Dose adjustment:
Asian p’ts: initial, 5 mg qd.
Concomitant cyclosporine: max. 5 mg qd.
Concomitant gemfibrozil, lopinavir/ritonavir: max. 10 mg qd.
Severe renal impairment (CrCl < 30 ml/min) non-dialysis: Initial, 5 mg qd; maintenance, max. 10 mg qd.
Contraindication:
P’ts with active liver disease or unexplained, persistent elevation of serum transaminases, pregnancy or breast feeding.
Adverse effect:
Common: abdominal pain, constipation, nausea, arthralgia, myalgia, asthenia, headache, urinary tract infectious disease, pharyngitis, influenza-like illness.
Serious: liver enzymes abnormal, disorder of muscle, rhabdomyolysis, rupture of tendon.
