適應症:治療陣發性上心室心搏過速,輔助診斷QRS波變寬或變窄之複雜性上心室心搏過速。

Usual dose:

Cardiac thallium 201 study: IV

Adult: 140 mcg/kg/min IV for 6 min.

Supraventricular tachycardia: IV.

Adult: initial 6 mg IV bolus over 1-2 sec, increase to 12 mg q1-2min as needed for 2 doses, max. 12 mg/dose.

Pediatric (< 50 kg): initial 0.1 mg/kg/dose IV as a rapid bolus, max. 6 mg/dose, may repeat at increasing increments of 0.2 mg/kg/dose as need, max. 12 mg/dose.

Paroxysmal supraventricular tachycardia: IV

Adult: initial 6 mg IV peripheral bolus over 1-2 sec, increase to 12 mg q1-2 min as needed for 2 doses; max. single dose 12 mg.

Pediatric

(< 50 kg): initial 0.05-0.1 mg/kg/dose IV as a rapid bolus, max. 6 mg/dose; may repeat at increasing increments of 0.05-0.1 mg/kg/dose IV q1-2 min as needed; max. single dose 0.3 mg/kg.

(≧50 kg): initial 6 mg IV peripheral bolus over 1-2 sec, increase to 12 mg q1-2min as needed for 2 doses; max. single dose 12 mg.

Contraindication:

Second or third degree AV block, sinus node dysfunction, such as sick sinus syndrome or symptomatic bradycardia.

Adverse effects:

Common: chest pain, lightheadedness, flushing, nausea, pain of head and neck region, headache, dyspnea.

Serious: bradyarrhythmia, cardiac dysrhythmia, incomplete atrioventricular block (Severe), bronchospasm in asthmatics.
