適應症:短期使用治療心室纖維顫動、Wolff-parkinson-white氏症候群、上室性及心室性心搏過速、心房撲動心房纖維顫動。用於對電擊具抗性的心室纖維顫動相關之心跳停止時所進行的心肺腹甦術。

Usual dose:

Supraventricular arrhythmia: IV/PO

Adult: 300 mg IV for 1 h, then 20 mg/kg for 24 h, then 600 mg PO daily for 1 wk, then 400 mg Po daily; maintenance, 200 mg PO daily.

Ventricular arrhythmia, life-threatening; treatment and prophylaxis: PO

Adult: initial, 800-1600 mg daily for 1-3 wks (for total daily doses 1000 mg or more or when gastrointestinal intolerance occurs, give in divided doses with meals), then titrate down to 600-800 mg daily for one mon.

Ventricular arrhythmia life-threatening; treatment and prophylaxis: IV

Adult: initial, 15 mg/min IV for 10 min, then 1 mg/min IV for 6 h, then 0.5 mg/min IV for 18 h; maintenance, 0.5 mg/min IV after the first 24 h with a concentration of 1-6 mg/ml.

Advanced cardiac life support: IV/Intraosseous

Adult: 300 mg by rapid infusion; if necessary, a supplementary dose of 150 mg by rapid infusion may be considered; max. daily dose 2.2 g.

Pediatric: pulseless ventricular tachycardia/fibrillation, 5 mg/kg by rapid IV bolus or Intraosseous injection; may repeat up to 15 mg/kg; max. single dose 300 mg.

Dose adjustment:

Hepatic failure: dosage adjustments may be required

Geriatric: dosage adjustments may be required; lower initial doses are recommended.

Contraindication:

Cardiogenic shock, hypersensitivity to amiodarone or to any of its components, including iodine, second or third degree AV block (if no pacemaker is present), severe sinus bradycardia, severe sinus node dysfunction.

Adverse effect:

Common: bradyarrhythmia, hypotension, photosensitivity, constipation, loss of appetite, nausea, vomiting, abnormal gait, coordination problem, involuntary movement, peripheral neuropathy, corneal deposit, visual disturbance, fatigue, malaise.

Serious: cardiac dysrhythmia, congestive heart failure,  shock, Stevens-Johnson syndrome, disease of thyroid gland, thrombocytopenic disorder, hepatotoxicity, pulmonary toxicity.
