IAMPO1 “Ampolin injection” 500 mg/vial

適應症:葡萄球菌、鏈球菌、肺炎雙球菌、腦膜炎球菌,及其他具有感受性細菌引起之感染症。

Usual dose:

Susceptible infections: IV/ IM

Adult: 0.5-3 gm q4-6h.

Pediatric: 100-200 mg/kg/day divided q4-6h; max. 12 gm/day.

(neonates≦1 wk):

(≦2 kg): 50 mg/kg/day divided q12h (meningitis, 100 mg/kg/day divided q12h.)

(≧2 kg): 75 mg/kg/day divided q8h (meningitis, 150 mg/kg/day divided q8h.)

(neonates≧1 wk):

(≦1.2 kg): 25 mg/kg q12h (meningitis, 50 mg/kg q12h)

(1.2-2 kg): 25 mg/kg q8h (meningitis, 50 mg/kg q8h)

(≧2 kg): 25mg/kg q6h (meningitis, 50 mg/kg q6h)

Prophylaxis – Procedure on GI tract: IV/ IM

Adult: 2 g plus gentamicin 1.5 mg/kg IV/IM 30 min prior to procedure.

Endocarditis; prophylaxis: IV/IM

Adult: 1-2 gm plus gentamicin 1.5 mg/kg IV/IM 30 min prior to procedure.

Pediatric: 50 mg/kg 30 min prior to procedure; add gentamicin 1.5 mg/kg in high-risk p’ts, with follow-up dose of ampicillin 25-50 mg/kg 6-8 h later.

Meningitis: IV/IM

Adult: 8-14 g/day divided q4-6h.

Pediatric: 200-400 mg/kg/day divided q4-6h; max. 12 gm/day.

Dose adjustment:

Renal failure: CrCl 10-50 ml/min, usual dose q6-12h; CrCl < 10 ml/min usual dose q12h.

Adverse effect:

Common: rash, urticaria, diarrhea, nausea, vomiting.

Serious: hypersensitivity reaction.
