OFOLI2 “Folina tablet” 15 mg/tab
適應症：超量使用葉酸拮抗劑時，消滅其毒性及抵消其作用、治療因斯潑盧營養缺乏、妊娠及其幼齡等引起之巨初紅血球性貧血。
Usual dose:
Adult: 15mg (10 mg/m(2)) PO q6h until serum methotrexate methotrexate level below 0.01 mcmol; if 24 h serum creatinine 50% above baseline, 24 h methotrexate level > 5 mcmol, or 48 h methotrexate level > 0.9 mcmol, increase dose to 100 mg/m(2).
Dose adjustment:
Diarrhea/stomatitis (moderate/severe): decrease 5-fluorouracil dose 20% (WBC/mm(3) nadir 1,000-1,900; platelet 25,000-75,000/mm(3)); decrease fluorouracil dose 30% (WBC/mm(3) nadir less than 1,000; platelet less than 25,000/mm(3)).
Contraindication: megaloblastic anemias due to vitamin B12 deficiency, pernicious anemias due to vitamin B12 deficiency.
Adverse effect:
Common: diarrhea, nausea, stomatitis, vomiting, fatigue.
Serious: allergic sensitization.
