適應症:狹心症、不整律(上心室不整律、心室性心博過速)、原發性及腎性高血壓、偏頭痛、控制原發性震顫、控制焦慮性心博過速、甲狀腺毒症的輔助劑、親鉻細胞瘤。

Usual dose:

Pediatric (neonate): 0.2- 0.5 mg/kg q6h.

Angina pectoris:

Adult: 80-320 mg daily (divided 2-4 times/day)

Congestive heart failure:

Adult: 30 mg tid

Pediatric: 1-2 mg/kg/day (in divided doses)

Hypertension:

Adult: initial, 40 mg bid, titrate to maintenance, 120-240 mg daily (divided 2-3 times/day), max. 640 mg daily (in divided doses).

Pediatric: initial, 0.5-1 mg/kg/24 h (divided q6-12h), titration, may increase dose at 3-5 day intervals; usual dose 2-4 mg/kg/24 h (in divided doses); max.16 mg/kg/24h.

Idiopathic hypertrophic subaortic stenosis:

Adult: 20-40 mg tid-qid.

Pheochromocytoma, Inoperable tumor:

Adult: 30 mg daily (divided).

Pheochromocytoma, operable tumor:

Adult: preoperative dose, 60 mg/day for 3 days prior to surgery, with alfa-blocker.

Postmyocardial infarction syndrome:

Adult: 180-240 mg daily (divided 2-4 times/day) for at least 3 yrs.

Thyroid storm:

Adult: initial, 10 mg qid, maintenance, 40-80 mg q6h; max.dose 120 mg qid.

Pediatric: neonates, 2 mg/kg/24 h (divided q6h), adolescents, 10-40 mg q6h.

Dose adjustment:

Hepatic failure, Geriatric: low dose titration is suggested.

Contraindication:

Bronchial asthma or chronic obstructive pulmonary disease, cardiogenic shock, hypersensitivity to propranolol, overt cardiac failure, second and third degree AV block, severe sinus bradycardia.

Adverse effect:

Common: atrioventricular block, bradyarrhythmia, cold extremities, dermatitis, pruritus, urticaria, loss of appetite, nausea, vomiting, excessive day and night-time sleepiness, impaired cognition, insomnia, paresthesia, depression, psychotic disorder, dyspnea, wheezing.

Serious: cardiorespiratory arrest, congestive heart failure, myocardial infarction, erythema multiforme, Stevens-Johnson syndrome, toxic epidermal necrolysis, asthma, bronchospasm.
